    Funeral Expenses Payment application forms

    Print out and complete the correct form to claim a Funeral Expenses Payment. Read the guidance notes before you fill in the form.

    Details

    If the deceased is a child, use Funeral Expense Payment application form (SF200 child). For the purposes of this form, a child is someone under 16, or someone aged 16 to 19 in full-time, non-advanced education or training.

    In all other circumstances, use Funeral Expenses Payments application form (SF200 adult).

    You can download and print the relevant application form below, the forms have notes to help you fill them in. You can also fill in the form online and print off your completed form. 

    Alternatively, printed versions of the SF200 adult and child forms and notes are available from your local Jobs and Benefits Office.
