Birth re-registration forms

To re-register your child's birth, you will need to fill in one of the following forms.

Details

fill in form GRO 12 if both parents want to record the father's name on the registration
fill in form GRO 15 if your child was born when you were unmarried or not in an opposite sex civil partnership and you have now married or formed an opposite sex civil partnership
