Maternity Allowance form (MA1), guidance notes and test period table

Download and send a completed MA1 form to the address on the form. You must use the right postage - a first class stamp is not enough.

Details

Your claim to Maternity Allowance will be decided on the work you have done in your Test Period. This is the 66 weeks up to, and including, the Saturday before the week you expect to have your baby.

Test Period table from August 2023 to December 2025

You can get the dates by using the online Test Period calculator. You enter the baby’s due date, and it works out the dates you need. Go to: Check your Maternity Allowance dates
