Declaration of paternity form

If you and your partner are not married and you want to be recorded on your child's birth registration but can't go to the signing of the register, you'll need to fill in a declaration of paternity form.
Details
The declaration must be made in front of someone with the authority to witness an oath, for example a Justice of the Peace (JP). You can find a JP or lay magistrate in your area by contacting the Northern Ireland Courts and Tribunals Service Management Support Branch.
