DVA will pay the appropriate fee to your GP (doctor) for the completion of your medical form(s) on the first occasion only. Payment for duplicates will be your responsibility.
Please download only one copy of the "Doctors claim for medical fee" and attach it to your medical form(s) before taking to your GP for completion. If you have more than one medical questionnaire to complete, one claim form is sufficient.
