Help with Health Service and Travel costs for low earners - Claim Form
You can make a claim by printing out the claim from HC1 Claim - for help with health and travel costs and filling it in.
When you have filled in the form you can post it to your local Social Security/Jobs and Benefits office. Alternatively you can hand it in at one of those offices or the·Benefit Shop in Royal Avenue Belfast.
